During the inspection there were several dead roaches in the dining area leading to the kitchen. The health inspector recommended a deep clean to remove the pests.

This is not the first time Teriyaki Express II had roaches. In September 2020 mice defecation and roaches were found during an inspection.

The meat marinade machine was dirty with food waste.

There were no paper towels at the hand sink and there were no paper towels in the  women's restroom.

The can opener was stored as clean, but was really soiled with food waste. The inside of the microwave was dirty with food splash. Additionally, the outside of the microwave had sticky residue on it.

Several foods were missing date labels, failure to correct date labeling will result in permit action against the Teriyaki Express II restaurant.

The inspector noticed several bulk containers of soy sauce stored under the prep table with no covers or lids.

Dirty sanitizer water in a bucket was being used for storing wiping cloths. The manager stated the sanitizer water was from the previous day.

In a past inspection a live roach crawled out from inside the chicken marinade machine right in front of the inspector.
